Provide the name of the source organization (the current registrant of the IP address blocks being transferred) exactly as it appears in the RIR Whois database. 03. Provide the original registration date for the IP address blocks listed in line 01. This can typically be found in the RIR Whois database. 04. Provide the name of the recipient (receiving) organization. Enter the full legal name of the organization to which the resources will be transferred. 05. Provide the full legal or postal address for the recipient organization. 06. Provide the first and last name of the contact person for the recipient organization. This is the person we may contact during the evaluation of the request. 07. Provide a phone number for the contact person listed on line 06. 08.
